How to Request Health Coverage Tax Documents
Due to recent changes in federal requirements, the university will no longer automatically mail Form 1095-C to employees on university medical coverage. Form 1095-C, which confirms that you were covered under health insurance, is not required to be filed with your tax return.
You may receive a copy of your 1095-C upon request. Please request one online here or by mail to Human Resources, 875 Perimeter Dr. MS 4332, Moscow, ID 83844. Email benefits@uidaho.edu or call 208-885-3638 with any questions.
