    Set up a Federal Student Aid Identification (FSA ID)

    Your FSA ID (username and password) is your legal, digital signature. Each student should create their own FSA ID, and parents should create their own FSA ID. Students and parents cannot share the same account username and password. Your FSA ID gives you access to certain information online and allows you to sign your FAFSA and promissory notes electronically.

    General Forms

    Release of Information/Consent for Release

    Completing and returning this form will allow financial aid staff to discuss financial aid matters with the individuals listed on the form. Persons granted information access are typically a spouse or parent.

    Students use this form to record a change to their enrollment plans (number of credits or semesters of enrollment).

    Consortium Agreement

    Students use this form only if they are taking classes at another institution but would like to receive credit for the classes and financial aid from University of Idaho. Please read the instructions below first.

    Consortium Agreement Form

    To receive Financial Aid from University of Idaho while attending both Idaho and another institution, you must be enrolled in at least 3 University of Idaho credits and have an approved consortium agreement each semester. To start this process, download the Consortium Agreement Form above and follow the instructions and process below:

    • Step 1: Student must complete the following sections: Student Information, University of Idaho Information, Student Certification, Course Information.
    • Step 2: Once the above sections are complete, the student must submit the form to the host institution for completion by the host institution’s financial aid office. The host institution is the college or university the student plans to attend while also enrolled at U of I.
    • Step 3: The host institution will complete their financial aid section and then send the form to the student and/or to consortium@uidaho.edu.
    • Step 4: Once received, U of I Student Financial Aid Services (SFAS) and Registrar will work together to determine eligibility based on cumulative credits.
    • Step 5: U of I SFAS will determine financial aid eligibility based on enrollment and award the student accordingly.

    University of Idaho/Washington State University Cooperative Course Program

    Students receiving their financial aid in a degree program at U of I and enrolled in University of Idaho/Washington State University Cooperative Course Program may take approved classes at Washington State University (WSU) and count those credits toward their financial aid eligibility. No additional forms need to be completed as course information will be shared electronically between the two institutions.

    The classes in the program make up less than 50% of any degree program at U of I. In most cases, students will just take 1 to 5 classes to supplement their degree requirements at U of I. Class will be held on one of the WSU campuses (most on the Pullman campus) and may be offered in a traditional classroom setting, online, or a combination of both.

    U of I cost of attendance will be used for calculating financial aid eligibility. Students will pay U of I base tuition and fees. Special additional class fees may be charged to the student by WSU depending on the class. If a student files an appeal for a special circumstance those extra fees may be considered in the student's cost of attendance based on the professional judgment of the financial aid office.

    Short Term Loan Application

    Short-term loans are designed to bridge an emergency shortfall between expected resources and payments that may fall due before a resource is available. Students use this form to apply for an Emergency Short Term Loan.

    Cooperative Education

    Cooperative Education is an educational model in which students alternate semesters of academic study with terms of full-time (paid) employment in positions related to their academic and/or career goals. This form is used to determine if a student wishes to utilize financial aid for the co-op semester or not.

    Petition for Financial Aid Reinstatement Form

    Students who have been placed on financial aid suspension will need to complete this reinstatement form. Once completed by the student, the form will be forwarded to the appropriate college contact for initial review before being sent to Student Financial Aid Services for final review.

    Parent PLUS Exceptional Circumstances Appeal Form

    Dependent students and their parent can use this form to appeal an extenuating circumstance which impacts their ability to borrow or repay a Parent PLUS loan.

    Dependent students and their parent(s) may use this form to appeal FAFSA information based on changes to your family’s income, and/or excessive out-of-pocket medical expenses.

    Students use this form to appeal FAFSA information based on changes to your income, excessive out-of-pocket medical expenses.

    Parent Change in Marital Status Form

    Dependent students and their parents may use this form to report a separation, divorce or widowed status that has occurred since the FAFSA was filed, a marital status that was mistakenly reported on the FAFSA, or to clarify marital status and income information if requested from Student Financial Aid Services.
