Faculty Evaluation of Administrators Form 2 Now Available Online

Faculty are given an annual opportunity to provide confidential feedback on their administrators (deans, associate deans, department chairs, program heads, etc.). This is important information for their supervisors as they prepare 2018 evaluations. Great care is taken to ensure comments remain confidential and are only viewed by the administrators' supervisor(s). Feedback for 2018 will be collected through online surveys. The information will be managed and distributed by the Office of Institutional Effectiveness and Accreditation.

Paper forms will also be accepted by the Provost’s office (campus zip 3152 or The form is available online.

This process is outlined in section 3320 of the Faculty Staff Handbook. The policy defining administrators’ responsibilities can be found in section 1420. General information about performance evaluations may be on the Office of the Provost and Executive Vice President’s website.

The deadline to submit feedback is 5 p.m., Friday, Jan. 18. Thank you in advance for your response.
