ALCP Airport Request Form

In order for the ALCP staff to assist new students when they arrive, such as picking them up from the Moscow-Pullman Regional Airport or assistance with reserving  a hotel room, ALCP needs this important information.

ALCP Certificate of Financial Responsibility 13-14

ALCP students are required to sign and verify that they have financial resources available to pay for all estimated expenses during their entire time of study.  The ALCP Certificate of Financial Responsibility MUST be filled out any time your funding for school changes or you request a new I-20/DS-2019.

SHIP Waiver Form

All students attending the University of Idaho MUST fill out the Student Health Insurance Waiver form. Students with health insurance that meets the minimum requirements, must provide proof of insurance. This form is due the first day of ALCP New Student Orientation.  Turn in the form and proof of insurance into Laila or Ashleigh in the ALCP office.

Health Insurance Alternatives

Students who do not want to purchase the University of Idaho's Student Health Insurance Program (SHIP), MUST purchase health insurance that meets the minimum requirements. Students must have health insurance for EVERY DAY they are in the United States. We encourage students to purchase health insurance from the list provided.  Students without health insurance will be charged for SHIP automatically.

Student Transfer-In Form

Students enrolled at a different language program in the United States, but want to enroll in the ALCP MUST complete the Student Transfer-In form, with the help of their current program's Designated School Official (DSO). Students must use this form and the ALCP Acceptance Letter to have their SEVIS record transferred to the University of Idaho, Moscow, Idaho.
