General Forms

Completing and returning this form will allow financial aid staff to discuss financial aid matters with the individuals listed on the form. Persons granted information access are typically a spouse or parent.

Consent for Release

Students use this form to record a change to their financial aid award (acceptance, increase or decrease), and/or enrollment changes (number of credits or semesters of enrollment).

Students use this form only if they are taking classes at another institution, but would like to receive credit for the classes and financial aid from University of Idaho.

Short-term loans are designed to bridge an emergency shortfall between expected resources and payments that may fall due before a resource is available. Students use this form to apply for an Emergency Short Term Loan.

Cooperative Education, is an educational model in which students alternate semesters of academic study with terms of full-time (paid) employment in positions related to their academic and/or career goals. This form is used to determine if a student wishes to utilize financial aid for the co-op semester or not. 

Parent Verification Forms

Documents for the 2022-2023 and 2023-2024 aid year are available through the student's VandalWeb.

Dependent students use this form only if they have been asked to verify their household size or the number of people in their parents' family who are in college.

Parents of dependent students should use this form only if they have been asked to download and submit.

Parents use this form only if they have been asked to verify amounts involved with cash, savings, and checking accounts, business value or debt, investment values and debts, or farm value and debts.

Parents use this form only if they are asked to verify the amount of untaxed income used in the prior tax year.

Parents who are required to provide income tax information must either use the IRS Data Retrieval Tool through the FAFSA, or provide a signed copy of the appropriate 1040, 1040A, or 1040EZ form.

Parents who did not file taxes but are required to provide income information must complete this form and return along with all applicable W2s.

Student Verification Forms

Documents for the 2022-2023 and 2023-2024 aid year are available through the student's VandalWeb.

If required, this form must be signed in-person and accompanied with your original identification document(s). If you are unable to come to our office, please call us at 208-885-6312 or email finaid@uidaho.edu.

Appeal Forms

Students who have been placed on financial aid suspension will need to complete the reinstatement form through Section D. The student and a designated contact from your college will help you complete Section E before the form is routed to Student Financial Aid Services.

Parent(s) of dependent students can use this form to appeal an extenuating circumstance which impacts their ability to borrow or repay a Parent PLUS loan.

Parents of dependent students use this form to appeal FAFSA information based on changes to your income, excessive out-of-pocket medical expenses, etc.

Parents use this form to report a separation, divorce or widowed status that has occurred since the FAFSA was filed, a marital status that was mistakenly reported on the FAFSA, or if requested from Student Financial Aid Services as clarification for income information.
