Certified Mail

Instructions for completing the US Postal Service Certified Mail Receipt

  1. Fill in recipient's name and address at the bottom of form and your department name and zip in the area above the Postage box. 

  2. DO NOT fill in postage and fees, Campus Mail and Receiving will do this for you and return the receipt to your department. If you do not need the postage and fees portion filled out, you may tear off and keep your receipt. 

  3. Peel backing from the bar-coded label and place at the top of the envelope immediately to the right of the return address so the dotted line is even with the top of the envelope. Fold the upper green part of the label over the top of the envelope. 

  4. Send to Campus Mail and Receiving. 

  5. Please dispose of old certified labels that do not have a barcode on them. The Post Office will no longer accept forms without the barcode.

Delivery status of certified and registered mail items, both inbound and outbound, may be verified via the US Postal Service website.
