The links below are for the most common 4-H activities waivers for use during 4-H events that all participants are covered by American Income Life Insurance.
If the activity or event you will be holding does not fall into one of the below categories, please contact Risk Management to have one prepared.
If NON 4-H members are participants, program providers must make arrangements for and purchase coverage for the NON 4-H member participants through UI Risk Management. Information and forms are available at Camp Insurance.
