Chemical Waste Collection Request Form

In January 2011, EHS implemented a new Chemical Waste Collection Request form.  Some of the significant changes include: 

• A simpler and more intuitive input page.
• You may now use Internet Explorer, Firefox, or Safari for Windows or Macintosh.
• Your first name, last name, department, phone number and
  e-mail address will automatically fill in from your login information.
• You select your building from a drop down menu.
• You do not have to search for room location; simply enter the room number where the chemical waste is located.
• The system will allow only one container per request; thus, each container will have a unique number assigned. You can no longer submit more than one container per entry but you do have the option of bringing forward container information to submit an identical container.
• You need to print the waste container label when prompted by the system and attach the label to the container.
• A receipt will be sent to the e-mail address that is associated with your login information.
