Forms

A protocol form must be submitted in electronic format to the IACUC (iacuc@uidaho.edu) for review for all projects utilizing live and dead vertebrate animals. The surgery form must be completed and submitted with any protocol in which a surgical procedure is being performed. A separate surgery form must be completed for each surgical procedure. Both forms are in the same file. 

Protocols in pain category C as described in the protocol form instructions may be submitted at any time for designated member review which requires 1-2 weeks from submission. Protocols in pain categories D or E require full committee review. In order for all committee members to have adequate time to review the materials prior to meeting the following submission deadlines will be strictly enforced.

  • Notification of Contract Services for Animal UseNotification of Contract Services for Animal Use is needed when using contract animals services with outside organizations, such as for antibody production.
    All contract services in which live animals are used to produce the desired service endpoint and are funded by federal agencies must be through companies with a valid PHS Animal Welfare Assurance. So that we have this confirmation on record, please use the link below to notify the ACUC of your intent to use such services. Be sure to include both the funding agency for your project and the contractor you intend to use.  Notify the ACUC
