Incident Report Form
- If you know, or even suspect, your computer has been compromised please report the incident to the Information Technology Services Security Group.
- If you believe another computer has been compromised please include log information, how you are collecting the log information, and the reason(s) for your suspicion.
Please note: contact information is not required although we would like to contact you by e-mail or phone depending on the type of issue reported.
