1. Change of Situation Form - Students use this form to record a change in award amounts (acceptance, increase or decrease) of their financial aid award, and enrollment changes (number of credits or terms of enrollment).

2. Work-Study Preference Sheet - Students who accept a Work Study award can click here to log in and select a position. Students who are unable to select a job through the on-line process cited above should contact Student Financial Aid Services for assistance.

3. Work-Study Job Change Request - Students use this form only if they have been awarded Work-Study and want to request to work in a different Work-Study position.

4. Release of Information Sheet - Students use this form to give someone else the right to talk to us about their financial aid information, typically, parents and spouses.

5. Consortium Agreement
- Students use this form only if they are taking classes at another institution, but would like to receive credit for the classes and financial aid from the University of Idaho. - Read these instructions first.

5a. Concurrent Enrollment -Some students are concurrently enrolled at North Idaho College (NIC) and the University of Idaho and want to receive financial aid through UI. Those students must go to the Financial Aid Office at NIC and have them complete a "Concurrent Enrollment Form" for each semester of concurrent enrollment. The form will be sent to the UI Student Financial Aid Services office to document the number of credits taken each semester so that financial aid can be awarded appropriately.

6. Short Term Loan Application - Short-term loans are designed to bridge an emergency shortfall between expected resources and payments that may fall due before a resource is available. Students use this form to apply for an Emergency Short Term Loan.

7. Federal Direct PLUS Loan Application - Parents use this form to apply for a Parent Loan for Undergraduate Students (PLUS) loan. Parents may borrow PLUS loan funds up to a student's cost of attendance minus all other financial aid resources. Awarding can occur as early as March, however credit checks for the academic year begin in June.  Click Here

16. Parental Appeal for Recalculation

25. Petition for Financial Aid Reinstatement Form - Students who have been placed on financial aid suspension will need to complete the reinstatement form through Section D.  The student and a designated contact from your college will help you complete Section E before the form is routed to Student Financial Aid Services.  The undergraduate SAP contact list is available here.

26. Work Study Change of Supervisor Form - This form is for Work Study delegates and supervisors who need to report a supervisor change for their current Work Study position. Completed forms can be emailed.
