Note: The LHSOM Travel Authorization Form includes travel at no cost to U of I and reimbursable travel. Contact Janice Todish at jtodish@uidaho.edu or 208-885-9083 if you have questions. Travel Pre-Authorization form MUST be submitted to Janice at least three (3) weeks prior to departure and MUST be fully approved before any expenses can be charged on your pcard or submitted for reimbursement.
NOTE: Service Contract Request form is for services and expenditures that exceed $1,000. This form is for faculty (not guest) to complete and forward to LHSOM Finance person.
Recital Audition Forms are required for undergraduate and graduate recitals. Paper copies of all rubrics are available in the LHSOM office.
Send the filled-out news release template to music@uidaho.edu, and it will be distributed to U of I Communications and to the local media.
