- Dr. Kozisek is a graduate of the Creighton University School of Medicine and completed his residency training at the University of Wyoming Family Medicine Residency in Cheyenne
- Dr. Kozisek practiced in Helena, Montana and served on the faculty of the University of Wyoming Family Medicine Residency at Cheyenne prior to moving to Boise in 1996 where he joined the Family Medicine Residency of Idaho as the Associate Director
- In 2007, Dr. Kozisek joined the St. Luke’s Health System and helped establish the Palliative Medicine Service, serving as its first medical director
- Dr. Kozisek is Board certified in both Family Medicine and Hospice and Palliative Medicine, and has completed the program in Palliative Care Education and Practice at Harvard University
- Dr. Kozisek is an Associate Clinical Professor with the University of Washington School of Medicine and enjoys working with medical students and residents on a regular basis. He has been a member of the Idaho admissions committee for UW for the past 6 years
