If you do not enroll within 30 calendar days of hire or job status change, coverage will be activated through the default selection as follows:
- Medical: High Deductible Health Plan- employee only
- Dental: Standard Dental- employee only
- Vision: Vision Perfect- employee only
- Life Insurance: 1x salary (base from employer)
- Short-term Disability: 50% coverage (base from employer)
- Long-term Disability: 50% coverage (base from employer)
- No pre-tax FSA
