Report Request Form

Instructions
    1. Fill out the request overview, report criteria, and output detail sections below. Items marked with * are required. Please be as specific as possible in defining the report. This will help us meet your specific needs in the quickest time possible.
    2. Output will normally be provided in a Microsoft Excel compatible format as an email attachment.
    3. Submit the request to Advancement Services by pressing the Submit button. You will receive and automated email response confirming your request has been submitted. We will contact you to request additional information as needed.
    4. Please contact us at 885-7091 if you have any questions.
** We will be short staffed for the next several months. Please allow up to 10 working days, except during peak periods or for special requests.
 
Request Overview
      Please fill out all fields. We can typically process requests within 3 working days, however, since report complexity and seasonal demand can vary, please try and anticipate this as you plan your programs. If you find yourself in special circumstances requiring a shorter Needed by date, please indicate the date and mark your request urgent by clicking the Urgent check box below and we will do our best to accommodate your request.

      Describe the group of people and/or organizations that the report should generate output for. (This does not define what you will see in the report, but the types of people or organizations in the report) Fill out all that apply, and be as descriptive as possible.
