Registration:

Use the registration forms to register for an ISI course. Use the online form to pay via credit card. Incarcerated students may use the PDF registration form to submit check, money order, or credit card payment.

Proctor Request:

If your course requires the use of a proctor, use the proctor form to request the use of a specific proctor. Proctor policies, selection information, and proctor requirements are listed on this form and should be reviewed before completing the proctor request.

Exchange:

Use the exchange form to request a course exchange for a different class. A course exchange is available within 45 days of registration. There are no exchanges after 45 days, regardless of the student’s progress in the course. One exchange per registration is allowed. The course completion date will remain the same.

Drop Request:

Use the drop form to drop an Independent Study in Idaho course and, when eligible, begin the refund process. A dropped course without a final grade will not appear on your academic transcript, and there will be no permanent academic record at any of the cooperating institutions.

Course Evaluation:

Use the course evaluation form to provide an evaluation of the course and the ISI program upon completion.

Course Enrollment Verification

If a college/university requires verification of enrollment, and you have not yet completed your course through Independent Study in Idaho, use this form to request a letter on ISI letterhead providing information regarding the course and current enrollment status. NOTE: Course Verification Letters are sent only to colleges/universities.

Catalog Request:

Use the catalog request form to request paper copies of our course catalog be sent to you. We send current catalogs twice a year to businesses, libraries, colleges, universities, and public schools upon request.

Student Information Update:

Use this form to change your contact information for use by the Independent Study in Idaho program.

• Student Name Change

  • If your legal name is different from ISI records, you must submit a Change of Name Form with supporting documentation to University of Idaho's Registrar's Office.

Consent of Release of Student Information:

Use the consent of release of student information form to give Independent Study in Idaho permission to share information about your academic progress in ISI courses and/or your ISI financial account. This is also known as a FERPA release form.
